Always start your letter of recommendation with the date in the top left corner.

Address the letter. Make sure to use the recipients proper title; Mr., Mrs., Ms., Dr., etc. If you do not know whom the letter will be going to simply address it as "To whom it may concern".

The first paragraph should be used to establish your relationship with the person you are writing the letter for and establish the purpose of the letter. Make sure that you specify the capacity of your relationship (friend/colleague) and the duration of your relationship and/or when it began.

The second paragraph of your recommendation letter should be used to describe the person whom you are writing for and back it up with examples. For example, if you say that Billy is compassionate you must describe the volunteer work he does. It is very important to be honest in this section and not over-sell your colleague.

The third paragraph is optional. This is the space where you should describe any concerns that you may have about the applicant. Give your honest opinion but make sure to state it tactfully.

The fourth paragraph should consist of your overall recommendation for the applicant. You should state that you believe the candidate is or is not a good match for the employer, and if so that you believe he/she will be able to contribute to the company, offer valuable insight, or gain valuable experience, etc. At the conclusion of the fourth paragraph you should encourage the reviewer to contact you if he should have any questions.

Sign the letter "Sincerely, Your Name" and leave space for your signature. Include your contact information including address, phone number, and email address underneath the signature.
